The BlueCard® program
Make sure your healthcare needs are met wherever you are located. Select health plans with this program have access to nationwide coverage and network providers.
A covered person or an authorized representative can submit written proof of any service, supply, prescription drug, test, equipment or other treatment up to 180 days after the service. In most cases, your healthcare provider will file claims directly with us for services provided. When that does not occur, you may submit the claim directly to us. You will need:
- Written proof of services
- A medical claim form
Send completed documents by mail, email or fax to the contact information listed on the claim form. For more information, call the customer service number on the back of your member ID card.
Important: Your submission may be returned if it does not comply with our filing and coding policies and procedures. You may request a copy of the claim coding policies and procedures from us or the service provider. For more information, call the customer service number on the back of your member ID card.
Register or sign in to Blueprint Portal. It's our member portal and app where you can view your member ID card*, estimate treatment costs, find care, view your claims and more. Download the app for on-the-go access to your health plan.
*Digital ID cards are not available to all health plans.
The good news is that most of our health plans provide a digital ID card option. Simply sign in or register for Blueprint Portal, our members-only website and app. If you need a replacement member ID card you can request it from a customer service representative.

- Sign in or register for Blueprint Portal to review your statement of benefits and health plan specifics under the Coverage & Copays tab.
- Use the online search tool to see which procedures are covered by your policy.

Use our Find Care and Cost tool in Blueprint Portal to search for providers and estimate costs for tests and procedures. You can also ask your provider for more detailed information, how much they will bill for your test or procedure and any Current Procedural Terminology (CPT) code that will be used.
When you sign in to Blueprint Portal, you will find personalized health plan information. You can:
- Find in-network clinics, pharmacies and providers
- Find and estimate costs
- View and pay claims
- Learn more about health programs and benefits available
- Sign up to receive member communications and documents digitally
Getting care from providers who participate in your plan’s network often saves you money. Generally, in-network services are provided at lower rates than out-of-network services.
If you receive care from a doctor or hospital that is out-of-network, you still benefit from having insurance. However, you won’t get the lower cost that we negotiate with in-network providers, meaning you will likely pay more out-of-pocket.
