Formularies and drug pricing lists


When you have pharmacy benefits through your health plan, filling a prescription at a participating pharmacy is simple. Important details are provided to the pharmacist right away, such as: 

  • Any potential harmful interactions of the medication about to be dispensed with any other medications that you may already be taking
  • Whether this medication may duplicate another medication you are taking
  • Whether the prescribed dosage or strength is appropriate for your age

If you have a health plan through your employer, you may not have pharmacy benefits through us and your formulary and drug search will not be listed below. Check with your employer or call the number on the back of your member ID card to learn more about your pharmacy benefits.

Three-tier medications formulary

The three-tier formulary is a listing of covered medications and the corresponding copay tier under which the medication is listed. The specific dollar amount of copay for each medication will vary depending upon your policy benefits, but the tier assignment for the medication will be the same for all members. In general, the three copay levels are: 

  • First tier: Almost all generic medications (lowest copay of all tiers)
  • Second tier: Preferred brand-name medications and other lower-cost, brand-name medications (mid-level copay)
  • Third tier: High-cost medications or medications classified as nonpreferred (highest level of copay)
