Case management and chronic condition management


Are you looking to take control of your health? Many of our health plans offer case management and chronic condition management programs, which could be important services for you. We focus on the whole person, ensuring you're getting help with health conditions, diseases and prescription medications. Some plans also offer behavioral health support and community resources to help you find counselors or therapists, peer support, getting a ride to appointments and more.

Case management

A program specifically for you 

Everyone is different. Why use a one-size-fits-all approach to reaching personal health goals? Our case management program works with you to understand your goals and help you create a tailored plan to improve your health. 

You'll work with the same case manager who may recommend health programs, resources and opportunities that empower you to make decisions based on where you are in your health journey. 

How a case manager can help 

In addition to recommending health programs, your case manager can help you: 

Prepare for appointments. A case manager will provide information and questions to ask your provider, so you feel confident and ready for a productive conversation. 

Improve your health. A case manager can help you discover new ways to stay healthy with preventive care options and better lifestyle choices. 

Find quality care. A case manager can ensure you're seeing the right providers at the right time and guide you through the complex healthcare system. 

Advocate for your health. A case manager can equip you with information to help you better understand conditions, treatment options and how to use your health insurance so you feel informed when making healthcare choices. 

Who is a good candidate for case management? 

 Here are some of the top reasons to participate in case management: 

  • You live with a chronic condition like diabetes, heart disease, etc.
  • You have trouble paying for prescription medication, housing, etc.
  • You visit the ER frequently
  • You need help staying healthy 

A case manager may call to see if you’re interested and to learn how the program could best support you based on your health history and needs. You can also get started by calling the number on the back of your member ID card and asking to be connected to a case manager. 

Chronic condition management

Transform your health

Managing a chronic condition can be complex and frustrating. Forty-five percent of all Americans have a chronic condition—and that number is growing. 

If you've been diagnosed with one of six health conditions, your health plan’s chronic condition management program will help you set goals and manage your condition. Not all members have access to this program, so it’s important to review your benefits to ensure what is available.  

Our program focuses on: 

  • Diabetes (adult and youth)
  • Asthma (adult and youth)
  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease (COPD)
  • Coronary artery disease
  • Renal disease

Call the number on the back of your member ID card and tell them you’re interested in learning more about the program. Your information will be reviewed by a case manager, who may then contact you about joining the program.

Once you're enrolled, you'll be assigned a case manager who will be your primary contact throughout your time in the program.  

The program is covered at no cost to members, which means you don't pay anything to participate.
