Clinic or organization application process


Clinics and organizations must be registered with Octave Blue Cross and Blue Shield before services may be billed on the individual provider’s behalf. This includes clinics and group practices affiliated with individual providers, such as primary care clinics, specialty clinics and billing groups.

Determine your eligibility for contracting

Confirm your clinic or organization is eligible for contracting and meets applicable credentialing standards and network participation guidelines.

Group NPI or EIN

You must have a group NPI or EIN and credentialed individual providers with an active license to link to the clinic or organization at the time of the application.

Review network participation guidelines

Practitioners requesting participation in our networks must agree to the network policies, procedures, terms and conditions and meet credentialing standards.

Complete the required forms

Complete the organization request for application (RFA-Organization). A network operations team member will reach out for additional information. You will need to complete our EFT form (provided to you after your RFA is accepted), provide a bank letter and W-9.
